Clinical information system acceptability factors : comparative evaluation Fance (HEGP) - Québec (CHUS)

Implementation of information technologies (IT) is a major concern for healthcare establishments. With the development of health systems, there is a growing need to integrate IT for healthcare delivery. The computerisation of the clinical processes addresses the objectives of efficiency and effectiveness in the management of the patient. Its success presupposes the adoption, use and internalisation of IT throughout the all dimensions of healthcare. This research aims: (1) to model the clinical processes, (2) to quantify the use of clinical information system (CIS), (3) to identify the individual determinants of CIS post-adoption, and (4) to analyse the expectations of the professionals. Our model of CIS acceptability is an integration of seven dimensions, derived from published theories and IS evaluation models. Questionnaires were administered to physicians and nurses in France (HEGP) and in Quebec (CHUS). Multiple regression and modeling by structural equations made it possible to test the causality relations. The results show that the use of CIS differs by profiles and by sites but the professionals seem satisfied with the CIS in both environments. The scores by dimensions of the model are considered relatively acceptable. Analyses demonstrate that post-adoption intention is determined by perceived usefulness and satisfaction. The confirmation of expectations influences satisfaction, perceived usefulness and perceived ease of use, compatibility and the users support. These results position the identification and the analysis of needs at the centre of the acceptability process of IT in healthcare
